Transitional Care Nursing
UVA Transitional Care Hospital is the first long-term acute care hospital (LTAC) in the Charlottesville area and one of only six hospitals of its kind in the state of Virginia. Our freestanding facility, located close to UVA Health’s main campus, allows nurses to provide extended care for patients who are recovering from surgery, weaning off ventilators or have complex medical conditions or wounds that require daily nursing, therapy, nutritional service and a physician. Here, you will collaborate in a team-based environment, gain experience with advanced technologies and procedures, and coordinate closely with clinical experts from a wide range of specialties while helping our patients recover and return home.
Home Care Nursing, Therapy and Allied Health
When advanced care needs to be delivered at home, UVA Health nurses, therapists and allied health professionals are there — bringing advanced care, therapeutics and education that promote our patient’s health, recovery and independence. Our home care teams are some of our most knowledgeable and clinical advanced providers, capable of working with a wide array of patient conditions ranging from acutely ill to chronically disabled to terminally ill. This integrated care team works together to bring academic medicine to our patients at home in the city of Charlottesville, as well as Albemarle, Culpeper, Greene, Madison, Orange, Louisa, Fluvanna, Nelson, Buckingham, Augusta and Rockingham counties.
